Cards, GI, and Pulm/CC are probably the most competitive of the IM fellowships. You make good money, but have to really work for it. Heme/Onc is competitive, as well. It offers a better lifestyle, though, because there are few true emergencies where you're called in during the middle of the night.In general Rad-Onc tends to be much better in lifestyle. this is highly decisive. Rad onc is somewhate more technical related, I think. You like call, rounding the wards, placing catheters and chemo -> heme/onc. You like controlled schedule in the clinic and radiotherapy -> rad/onc. These are fairly different types of subspecialties - one is much more cerebral (as you pointed out) while the other can be much more procedural. Heme/onc can also be much more outpatient focused, possibly also with a better lifestyle.
