This is what I've got so far, up to the "new MidiQOL" line, it seems to work to detect the vulnerability, I just need to get it to apply extra damage.

You can put an active effect on the character that reminds you to roll saves vs poison with advantage. It works with Midi but it really only works if you're not fast-forwarding rolls since it's adding a little message to the dialog. If you're fast-forwarding rolls then that dialog is skipped. It's an option.
